Good morning BC's!! Here are the forms that I created for the new employee folder. If anyone sees something I missed or way to improve it; please let me know! A couple of things that I added in my folders that I did not put in the attachment are the Wage Supplement & Certificate of Achievement Stipend Program Flier, and our Flier for CCR&R. I also will be including the Thrive Assistant Form for Tele-health for any of my home-based providers who are Thrive members.
